Job Description

Customized Assistance Services (CAS) help Human Resources Administration (HRA) clients with health and/or mental health problems reach their highest attainable level of functioning and self-sufficiency by providing comprehensive, integrated, individualized clinical and support services. CAS works with other components of HRA and with other governmental and non-governmental service providers to create new programs and to integrate and refine existing services so the people it serves can achieve their maximum functional capacity. The program provides a continuum of integrated individualized services including biopsychosocial assessments, Federal disability benefits assistance, wellness planning, case management and vocational rehabilitation through education/training, job placement and retention services. WeCARE results in significant cost savings for the city by helping CA clients to obtain Federal disability benefits and employment.

Under the general supervision of the We CARE Operations Deputy Director with latitude for independent judgment, decision-making and action, the We CARE Program Support Specialist functions as an out-stationed liaison at We CARE vendor locations within Region I (Bronx, Manhattan and Staten Island) and Region II (Brooklyn and Queens). Representing the Human Resource Administration (HRA) and utilizing clinical expertise. The We CARE Program Support Specialist provides social work support services at either the primary We CARE vendor or subcontractor(s) location, facilitating the exchange of information between HRA Family Independence Administration (FIA), CAS Office of Business and Support Services (OBSS), We CARE Operations, and vendor staff. Provide clinical and technical assistance on HRA and We CARE procedures, HRA systems, and employment services guidelines.

Customized Assistance Services (CAS) is recruiting to hire one (1) Consultant Public Health SW, to function as WeCARE Program Support Specialist who will:

- Provide service interventions for We CARE clients and clinical advice to We CARE vendor/subcontractor staff on the provision of services to clients with limitations to employment.

- Perform Quality Assurance Reviews, reports, evaluates clinical information and provides recommendations to We CARE Operations Deputy Director for improving service delivery to We CARE clients at vendor/subcontractor sites.

- In conjunction with We CARE Operations Deputy Director, develop and complete reviews of service gaps in meeting the needs of the We CARE program participants, documents and formulates program improvement recommendations to senior leadership.

- Provide liaison services, linking vendor/subcontractor staff with HRA departments providing referral and follow-up services for clients with special circumstances.

- Provide clinical guidance to the vendor’s case management staff and ensures that client services are completed in a timely and appropriate manner and documented in HRA’s case record systems.

Minimum Qualifications

Graduation from an accredited graduate school of social work as evidenced by a master's degree or certificate and four years of full-time paid experience in public health, medical or psychiatric social work, at least two years of which must have been in a supervisory, administrative or consultative capacity.
Within 18 months of the date of appointment, all candidates will be required to obtain a certificate as Certified Social Worker (CSW) issued by the New York State Department of Education.
