Our Agency’s five strategic priorities, building off a recently-completed strategic planning process emerging from the COVID-19 emergency, are:
1) To re-envision how the Health Department prepares for and responds to health emergencies, with a focus on building a “response-ready” organization, with faster decision-making, transparent public communications, and stronger surveillance and bridges to healthcare systems 2) Address and prevent chronic and diet-related disease, including addressing rising rates of childhood obesity and the impact of diabetes, and transforming our food systems to improve nutrition and enhance access to healthy foods
3) Address the second pandemic of mental illness including: reducing overdose deaths, strengthening our youth mental health systems, and supporting people with serious mental illness
4) Reduce black maternal mortality and make New York a model city for women’s health
5) Mobilize against and combat the health impacts of climate change

PROGRAM AND JOB DESCRIPTION:

BEHS is seeking Prevention Outreach Specialists (POS) who are outgoing, detail-oriented individuals with strong interpersonal, computer and customer service skills, to support the Patient Engagement Call Center and NYC Care Calls Program. The Patient Engagement Call Center supports primary care providers and Community Based Organizations (CBO) in connecting eligible patients to evidence-based interventions to prevent or manage chronic disease. Intervention programs can include the National Diabetes Prevention Program, Diabetes Self-Management Education and Support (DSMES), and NYC Care Calls.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:


**AMENDED- The POS will participate in program implementation, engage with community members in multiple settings, and collaborate with community partners.
- Maintains ongoing contact with patients from community-based organizations (CBO’s) and primary care practices that are involved with providing services to support the diabetes self-management programs.
- Works with various departments and community groups across NYC to support and assist with community education, training, and health workshops.(AMENDED)6/27/23


Utilize effective communication strategies to describe available evidence-based programs such as the National Diabetes Prevention Program, Diabetes Self-Management Education and Support (DSMES), and NYC Care Calls to eligible patients including those that speak a language other than English
Coordinate with manager and team members on program evaluation
Collect and record accurate program data into databases
Prepare documents and reports of program progress as assigned
Participate in quality improvement to improve program volume and recruitment and accurate reporting
Maintain confidentiality of patient and program information per HIPAA and program protocols
Follow program protocols, implement telephone self-management support to a caseload of participants with type 2 diabetes including those that speak a language other than English; this involves a series of 6 telephone calls over a year
Use program protocols, plan tailored approaches to support participants to understand and implement their diabetes treatment plans to improve clinical outcomes and reduce disease-related emotional distress
Monitor and evaluate progress of assigned caseload of participants
Participate in quality assurance to ensure the program is delivered according to established protocols
Escalate cases to program leadership when barriers prevent patients from achieving desired health goals
Make recommendations on recruitment and retention procedures, and on curriculum implementation


Minimum Qualifications

1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.


Preferred Skills

- Bachelor’s Degree in health, health education, biological or social sciences - 2 or more years of related healthcare or case management experience - Experience in health promotion or education - Strong written and oral communication skills required - Excellent interpersonal, collaborative and team skills - Ability to work independently and exercise a high degree of initiative to accomplish tasks and solve problems - Excellent time management and organizational skills - Strong computer skills: MS office - Willingness to work a non-traditional schedule (e.g 12pm-8pm) to accommodate schedule needs of program participants - Occasional work on weekends may be required - Verbal and written fluency in a language spoken by target communities such as Spanish, French Creole, or Bengali.
Residency Requirement

New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County. To determine if the residency requirement applies to you, please discuss with the agency representative at the time of interview.
