The Administration for Children’s Services (ACS) protects and promotes the safety and well-being of New York City’s children and families by providing child welfare, juvenile justice, and early care services. In child welfare, ACS contracts with private nonprofit organizations to support and stabilize families at risk of a crisis through preventive services and provides foster care services for children not able to safely remain at home. Each year, the agency’s Division of Child Protection responds to more than 50,000 reports of suspected child abuse or neglect. In juvenile justice, ACS manages and funds services including detention and placement, intensive community-based alternatives for youth, and support services for families. ACS is also a key part of NYCs early childhood and education continuum, providing childcare assistance to thousands of child welfare involved and low-income children so they can access safe, affordable, quality care.
The Administration for Children's Services (ACS) Office of Child and Family Health (OCFH) delivers direct medical healthcare that is comprehensive and appropriate, oversights health care delivered by its contracted external Voluntary Foster Care Agencies and monitors the appropriateness of services delivered to children and youth who require the level of care provided by short- and long-term health care facilities. The Nurse Clinical Monitor will be responsible to work with the ACS Special Care Unit (SCU) to monitor a case load of children and youth who reside in skilled nursing facilities or residential schools.
Duties will include, but are not limited to:
-Request and review medical, mental health and ancillary health service records from SCU Case Planners, health-related facilities, and residential schools to ensure compliance with established
health care standards.
-Use agency developed tool to determine compliance with all Federal, State, and City requirements.
-Participate in the skilled nursing facility placement authorization process for children/youth in foster care who require admission to a health care facility.
-Coordinate and conduct face to face and facility site visits regularly as determined by the child’s health needs to monitor the child’s progress, the quality of services received at the facility and the
overall environment of the facility.
-Conduct hospital visits for children on the assigned case load as needed.
-Participate in Service Planning Reviews (SPR), quarterly treatment meetings, Family Team Conferences, and discharge planning conferences.
-Maintain and document all internal SCU and ACS contacts, external facility and agency contacts and monitoring activities for assigned case load.
-Maintain and update assigned case files and logs
-Develop and regularly update medical summaries and document all follow up activities for all children and youth on assigned caseload.
-Complete service plan review and site visit reports timely
-Identify and screen case reviews for children with special medical needs and assist Nurse Supervisor to identify training needs and develop and evaluate corrective action plans
-Other duties as assigned
1. A valid New York State Registered Nurse License.
The preferred candidate should have experience with Pediatric/adolescent nursing, medical audit/ electronic medical record chart extraction, and Pediatric mental health nursing experience.
New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County.
