The Bureau of Chronic Disease Prevention (BCDP) strives to reduce the burden of chronic disease, including heart disease, obesity, and cancer, among New Yorkers. We work to address the impact of structural racism and other injustices that are a root cause of the inequitable prevalence, treatment, and outcomes of chronic diseases in communities of color and among other marginalized communities. BCDP focuses on nutrition, tobacco use, the built environment, screening for cancer, and reducing the incidence and impact of hypertension. BCDP works with partners in government and the community to employ evidence-based policies, programs, communications, and research aimed at shifting environments, changing systems, and promoting health equity. The Bureau sits within the Center for Health Equity and Community Wellness.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

Key responsibilities include: Assist with outreach to local stakeholders within neighborhoods with the highest prevalence of hypertension including drafting correspondence and sending emails. Ensure accurate and timely information is communicated.
Assist with community engagement efforts including facilitating community-based hypertension trainings and related task.
Enter and retrieve data via the Health Department's partner management Salesforce database for tracking communication and engagement
Contribute to program planning by sharing ideas and conducting background research.
Monitor program email account and respond to messages or forward them to other team members as appropriate.
Assist in planning for stakeholder convenings, including logistical tasks such as securing space and services.
Support implementation of program activities, including collecting data, providing information and assistance to coalition members, and connecting organizations with each other as needed.
Support ongoing relationships and collaborations with other Health Department programs working on hypertension.
Support Program Evaluator, in creating surveys utilizing online survey tools to help design evaluation forms, and pre- and post-tests and ensure responses are collected and organized for analysis.
Contribute to reports and other documents summarizing relevant information and data on, program plans and progress, and hypertension in NYC.
Assist with building relationships and engaging with stakeholders As needed, represent the program at community or key stakeholder events.
Assist with other projects and tasks as needed.


Minimum Qualifications

1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.


Preferred Skills

Bachelor's degree in public health or a science or social science field Excellent interpersonal and oral and written communication skills, including the ability to communicate effectively and work well with a wide variety of partners and stakeholders Strong organizational skills, with the ability to handle multiple and diverse assignments efficiently Proven writing skills, with the ability to shape messages for a variety of audiences Proficiency and prior experience using Microsoft Suite as well as other online tools and software Ability to exercise good judgment, take initiative and work independently Lived experience or experience working with marginalized communities in NYC that historically have had high rates of poverty and limited access to resources Commitment to and connection with NYC communities that experience avoidable and unjust differences in health outcomes Experience working in multi-cultural settings with racially, ethnically, and socioeconomically diverse communities.
Residency Requirement

New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County. To determine if the residency requirement applies to you, please discuss with the agency representative at the time of interview.
