The HEAT Coordinator will be responsible for:
- Utilize clinical experience with homelessness, mental health, substance use and/or criminal justice system to enhance client engagement and support
- Significant community-level work with community-based organizations, local business, community members and other assets to inform and improve interventions
- In conjunction with a partner, HEAT Navigator, the HEAT Coordinator will assist in client needs assessment, and will provide short term case management services
- Work in tandem with the HEAT Coordinator to assist clients in reaching their health-related goals
- Prioritize individuals who are identified as high utilizers of 911 behavioral health calls (3 + calls a month that result in hospital transport)
- Offer direct community engagement, prioritizing neighborhoods with the highest rates of behavioral health calls to 911, drug-related hospitalizations, psychiatric hospitalizations, and rates of incarceration.
- Maintain accurate software-based documentation of service objectives and outcomes as well as other services in accordance with DOHMH and program guidelines
- Attend program-related community, coalition and committee meetings
- Prepare reports as needed
- Participate in all program-related and supervisory meetings.

- Licensed professional in the areas of social service, health and human services, or another appropriate field - 2 years of experience providing support & services to vulnerable populations, particularly the homeless and people with health, mental health, and substance abuse issues - Experience navigating NYC’s mental health, substance use and criminal justice system, - Familiarity and openness to the principles of harm reduction - Ability to work effectively as part of a team - Willingness to travel within the five boroughs - Maintain appropriate professional and client level relationships - Proficient in the use of computers and software - Creative problem-solver who enjoys working on a fast-paced team, is highly motivated and able to coordinate multiple priorities - Ability to respond quickly and effectively to crisis situations - Ability to work independently and as part of a team and exercises mature and professional judgment - Strong com
