We serve as the population health strategist, and policy, and planning authority for the City of New York, while also having a vast impact on national and international public policy, including programs and services focused on food and nutrition, anti-tobacco support, chronic disease prevention, HIV/AIDS treatment, family and child health, environmental health, mental health, and racial and social justice work, among others.
Our Agency’s five strategic priorities, building off a recently-completed strategic planning process emerging from the COVID-19 emergency, are:
1) To re-envision how the Health Department prepares for and responds to health emergencies, with a focus on building a “response-ready” organization, with faster decision-making, transparent public communications, and stronger surveillance and bridges to healthcare systems 2) Address and prevent chronic and diet-related disease, including addressing rising rates of childhood obesity and the impact of diabetes, and transforming our food systems to improve nutrition and enhance access to healthy foods
3) Address the second pandemic of mental illness including: reducing overdose deaths, strengthening our youth mental health systems, and supporting people with serious mental illness
4) Reduce black maternal mortality and make New York a model city for women’s health
5) Mobilize against and combat the health impacts of climate change

PROGRAM AND JOB DESCRIPTION:
The mission of the Bureau of Public Health Clinics (BPHC) is to promote a healthy community by providing New Yorkers with the resources needed to make informed and empowered health decisions; identify and treat tuberculosis, provide immunizations, and sexual health services regardless of ability to pay or immigration status. To achieve this, BPHC provides direct clinical services at eight Sexual Health Clinics and three Tuberculosis Chest Centers; monitors disease trends; conducts outreach to providers and community groups; conducts research; and develops polices to improve sexual health and wellness and reduce racial inequities.
The Bureau seeks to hire a Community Coordinator/CBO Liaison.
BPHC's Community Engagement Unit oversees all external-facing efforts in BPHC including community outreach and facilitation of educational workshop in order to reduce inequities in STI's and TB by ensuring communities most impacted by those conditions are utilizing these services available in their community.
DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:
Community Engagement Unit Goals:
Garner community trust to collect community feedback data through various survey tools and use findings to inform strategy
Elevate community concerns related to public health clinic services.
Inform community outreach strategies.
Assist with other responsibilities as assigned.
Coordination of Community Engagement Unit Activities:
Coordinate activities led by the Community Engagement Unit, including assist with oversight and coordination of CBO Liaison schedules assigned to outreach activities throughout NYC.
Assist in community level outreach to promote new rollout of services and ongoing clinic services.
Coordinate with internal units and senior leads to distribute educational material (brochures, flyers, pamphlets, cards, etc.) about public health clinic services that include, sexual health, reproductive care, tuberculosis treatment and care, and immunization services.
Assure liaison calendar is on schedule on track and completed in a timely manner.
Assist with other responsibilities as assigned.
Overall Community Engagement Unit Activities:
Identify presenters and/or conduct presentations for BPHC staff on current developments in the public health clinics, that include but not limited to sexual health, reproductive care, tuberculosis treatment and care, and immunization services.
Coordinate with partner organizations to distribute educational materials related to public health clinic services. Participate in and help coordinate public events such as health fairs, job fairs, and other tabling opportunities.
Conduct quarterly quality assurance of outreach data.
Assure community outreach and engagement incorporates language needs of unit's outreach efforts.
Collaborate with bureau's implementation unit to identify funding streams and assist with writing of proposals.
Manage database of community partnerships and activities related to those relationships, such as Salesforce's PHPC.
Inform and/or manage community engagement data visualization/dashboard for reporting to senior leads and Assistant Commissioner.
Assist with other responsibilities as assigned.

1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.
