Job Description

The office of Adult Protective Service (APS) is mandated by the State of New York to provide social, legal and protective services to individuals 18 years of age and older, who because of mental or physical impairments are unable to meet their essential needs for food, shelter, clothing or medical care, secure benefits and entitlements or protect themselves from physical or mental injury, neglect , maltreatment, or financial exploitation; are in need of protection from actual or threatened harm, neglect or hazardous coordination; and have no one available who is willing and able to assist them responsibly.

APS establishes appropriate safeguards to protect a given individual's resources, safety, or health, and stabilizes the situation. APS ensures that medical and psychiatric services, eviction prevention, financial management, home care, legal services and as last resort, institutional placements are provided.

Adult Protective Services Program Staff nurses conduct thorough healthcare assessment of clients supporting APS caseworkers and supervisors on complex cases. Staff complete in-home healthcare evaluations for vulnerable individuals who require APS intervention. The staff make linkages to appropriate healthcare providers (including 911) to promote wellness, provide consultation and recommendations to social service staff at HRA request and participate in the Multi-Disciplinary Teams to impart medical expertise in critical cases

The office of Adult Protective Services is recruiting for one (1) Staff Nurse to function as the APS Staff Nurse in APS Manhattan South Office who will:

- Conduct in-home healthcare assessments, including physical health, mental status and cognitive (thinking) skills. Refers clients who are medically non-compliant to appropriate providers and
social work staff. Evaluates signs of abuse and make the appropriate medical referrals. Collaborates with APS caseworkers and external entities to arrange appropriate hospitalization through
911.

- Evaluate client’s abilities to perform activities of daily living, i.e. hygiene, toileting dressing, shopping, managing finances and managing medications, and the degree to which the client requires
assistance for each function.

- Identiy client’s medical needs and outline goals and interventions to achieve wellness. Develops a plan of care for the type of home care service to be provided to the client after assessing the
client and reviewing the case documentation.

- Gather information for medical histories and complete medication reviews, consulting with medical providers when necessary.

- Conduct assessment s of clients transitioning to a Nursing Home Facility and completes the PRI (Patient Review Instrument) for placement in long-term care facilities.

- Provide in-service training in healthcare issues for APS caseworkers and supervisors.

Salary Range: $ 100,750

Work Location: 400 8th avenue 7th floor

Hours/Schedule: 9:00am-17:00pm (Monday – Friday)


- Maintains the APS.Net database to reflect all activity, documentation and services provided to APS clients and maintains accurate daily logs and records and any other databases which reflect monthly reports, outcomes, and statistics.


Minimum Qualifications

1. A valid New York State Registered Nurse License.

- Critical Thinking - Working experience with APS or similar served population, such as individuals who struggle with mental illness, cognitive impairment, developmental disabilities and/or substance abuse - Strong interpersonal skills: effective and creative leadership ability with the capacity to work both independently and cooperatively to effect key strategies. - Excellent verbal and written communication and presentation skills. - Ability to work independently and within a fast-paced environment. - Working knowledge of HRA systems

Residency Requirement

New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County.
